The Access and Review Fingerprint Card is used by the Plainfield Police Department when an individual is requesting a review of their criminal history regardless of conviction status.

The response from the Bureau of Identification will be accompanied by a Record Challenge Form which a person used to record items in a record to be challenged. The contents of the form will contain sufficient information to identify the individual and the transaction referred to, and adequate space will be provided for the individual to record the facts of the challenge to file an administrative appeal complaint.

If a person feels that this process would be beneficial to them, they need to come to the police department and have fingerprints taken to be sent to the Bureau of Identification.
